Enroll for LUXEYE VISION PLAN by filling out the enrollment form below.
Once completed, head over to the EMPLOYEE ENROLLMENT page to submit your employee information.
Once everything is submitted, you and/or your employees will receive LUXEYE VISION card within 7-14 business days!
